What is a penis sleeve?
A penis sleeve — also called a penis sheath, cock sleeve, or extender — is a hollow, wearable device that fits over the penis during sex. It sits over the shaft (and sometimes the glans) and is held in place by the wearer's erection, a ring at the base, or a harness attachment. The external surface — the part that enters a partner — features the sleeve's texture, size, and shape rather than the wearer's own anatomy.
The result is a toy that does several things at once: changes sensation for both partners, adds girth or length, allows penetrative sex when erections are partial or absent, and introduces texture or vibration that wouldn't otherwise be present. It's one of the most versatile categories in the toy market — and one of the least talked about.
A sleeve changes what both partners feel. That's the point — and it's also why the right fit matters so much.
How does it work?
The wearer slides the sleeve onto their penis. Most sleeves are open at the base and closed (or shaped) at the tip. Once in place, the sleeve becomes the surface that a partner interacts with — its texture, girth, and length are what's felt internally, not the wearer's skin directly.
Internally, the wearer still experiences pressure and friction — particularly from the base ring and the snug fit of the sleeve around the shaft. Vibrating sleeves add motor stimulation felt by both partners simultaneously.
Types of penis sleeves
Extension sleeves
Add length beyond the wearer's own penis. Open at the base, closed at the tip with a realistic or textured head. Best for couples where one partner wants more depth of penetration.
Girth sleeves
Add circumference rather than length. The sleeve is wider than the wearer's shaft, so a partner experiences a fuller feeling without extra depth. Popular in the Perfect Fit Fat Boy line.
Hollow sleeves / strap-on sleeves
Fully hollow with no internal stimulation for the wearer — designed for people with partial erections or ED. The sleeve is worn over a flaccid or semi-erect penis (or over a harness) and held in place by a base ring or harness. These are covered in depth in our penis sleeve for ED guide.
Vibrating sleeves
Include a bullet vibrator in the tip or integrated into the sleeve body. Creates simultaneous vibration for both partners during use. Covered in our vibrating sleeve guide.
Textured sleeves
Add ridges, nubs, ribs, or other surface textures to the exterior. Primarily for the receiving partner's pleasure. Many basic sleeves are in this category.
Materials explained
If using a sleeve with multiple partners, choose non-porous medical-grade silicone. Porous materials harbour bacteria even after washing and cannot be fully sterilised.
Sizing and fit
Sleeve sizing has two dimensions most buyers miss: inner diameter (must accommodate the wearer comfortably) and outer dimensions (what the receiving partner experiences). A sleeve that's too tight on the wearer cuts off sensation and circulation. Too loose and it won't stay in place.

How to use a penis sleeve
Cleaning and care
Non-porous silicone sleeves: wash with mild soap and warm water, or use a dedicated toy cleaner. Allow to dry completely before storing. For TPE/TPR sleeves: wash thoroughly but understand these cannot be fully sterilised.
Store away from direct sunlight and away from other toys — some materials react chemically when stored in contact.

Frequently asked questions
Do penis sleeves reduce sensation for the wearer?
Somewhat — the sleeve acts as a barrier between the wearer's skin and their partner. Most wearers still feel pressure and friction, especially at the base. Some find this extends endurance, which they prefer. Vibrating sleeves compensate with direct motor stimulation.
Can you use a penis sleeve without an erection?
Hollow sleeves are specifically designed for this — they maintain their shape independently of the wearer's erection. Standard sleeves need some degree of firmness to stay in place. See our ED guide for hollow options.
Will my partner feel the difference?
Yes, noticeably. Texture, girth, and length all change what a partner feels. This is usually the primary reason people use sleeves.
Is lube required?
Strongly recommended. Without lube, the sleeve can create uncomfortable friction for both partners. Water-based lube is safe with all sleeve materials.
